Why structure beats headline rate

The tirzepatide label titrates from 2.5 mg through 5, 7.5, 10 and 12.5 to 15 mg over about 21 weeks. The starting dose is explicitly a tolerance dose and is not intended to produce weight loss. Most patients therefore spend the overwhelming majority of their treatment at a dose well above the one their price was quoted at.

On a tiered programme the bill follows them up. On a flat programme it does not.

The arithmetic

Brand Zepbound direct from LillyDirect: $299 at 2.5 mg, $399 at 5 mg, $449 from 7.5 mg upward — and the rate holds only if each refill lands within 45 days. Following the label schedule, that is roughly $5,088 across a year.

A flat-rate compounded plan at $186 is $2,232 across the same year. The $2,856 difference comes entirely from dose structure. Neither provider's advertised monthly figure predicts it.

These are different products with different regulatory status, so this is not a like-for-like recommendation. The mechanism is the point: where two programmes are otherwise comparable, structure decides the annual cost.

Escalates: Shed states its compounded programmes increase at higher doses. MEDVi is reported to reach $499 at 10, 12.5 and 15 mg. Both brand manufacturers tier by dose.

When tiered wins

If you stay at a low dose. A tiered programme's starter rate can be genuinely lower than a flat rate, and a patient maintaining at 5 mg may never reach the tiers that make it expensive. The problem is that you cannot know your maintenance dose at signup, which is the argument for treating the maintenance price as the real one.

Watch for caps as well as tiers

Some programmes cap the maximum dose rather than charging for it. One compounded semaglutide programme in our dataset is capped at 0.6 mg against the 2.4 mg used in the STEP trials. That is a lower-dose treatment, not a cheaper version of the same one, and it should not sit in the same ranking.
