Monthly vs Prepaid GLP-1 Programs
Prepaid twelve-month plans cut 12 to 71 per cent off the monthly equivalent depending on provider, and commit the full amount before you know whether you tolerate the medication. Month-to-month costs more and keeps the exit open. With a live FDA proposal affecting compounded supply, the refund position on a prepaid balance is worth getting in writing.
The gaps, provider by provider
Every provider in our dataset that publishes both structures charges more for monthly billing. The size of the premium varies enormously.
NexLife (opens nexlife.us in a new tab): $215 month-to-month against $186 on twelve months, a 16% premium, both verified. Enhance.MD: $329 against $280, an 18% premium. Shed: $349 against $245, a 42% premium. Henry Meds injectable semaglutide: $297 against $197, a 51% premium. Found: $289 against $169, a 71% premium and the widest in the dataset. These Henry Meds figures were taken from third-party publications and are withdrawn pending a capture of our own; see COR-009.
A comparison that quotes only prepaid rates therefore understates what a monthly patient pays by anywhere from a sixth to two-thirds.
What prepaying actually risks
Tolerability. Gastrointestinal effects are common during escalation, and in SURMOUNT-1 seven per cent discontinued at the 15 mg dose because of an adverse event. Committing twelve months before your first injection is a bet made without the information that settles it.
Regulatory continuity. The FDA has proposed excluding tirzepatide from the 503B bulks list, with the comment period closing 30 July 2026. Any compounded prepayment is exposed to that outcome, and very few providers publish what happens to a balance if supply is interrupted.
A moving market. Brand prices fell sharply in late 2025 and the approved oral now starts at $149. A twelve-month commitment forecloses a market that is still changing.
When prepaying is the right call
When you are already established on treatment and tolerating it. The withdrawal evidence points toward indefinite continuation, so a patient six months in is not really making a twelve-month bet — they are pricing something they were going to buy anyway.
And when the gap is large. A $120 monthly difference is $1,440 a year, which is real money for a decision you had already made.
Three answers to get in writing first
- What is refunded if I stop for a clinical reason, and on what timeline?
- What happens if supply is interrupted for regulatory reasons?
- Does the rate hold if my dose or formulation changes?
A provider who answers all three plainly is worth prepaying. One who does not has told you how the balance would be treated.
