Three-month prepaid plans sit between month-to-month flexibility and the twelve-month rates that headline most comparisons. They typically save 8–12% against the monthly rate while committing a quarter rather than a year — which matters because the first three months are when most discontinuation happens.
Why the middle term is the interesting one
Comparison sites rank on the twelve-month figure because it is lowest. Providers advertise it for the same reason. What neither emphasises is that the twelve-month rate requires committing before you know the two things that determine whether you will still be taking this in a year: whether you tolerate it, and what dose you settle on.
In SURMOUNT-1, 4.3% to 7.1% of participants discontinued because of an adverse event depending on dose, and that discontinuation clusters during titration — the first three to five months. A three-month term covers roughly the period in which you find out.
What the term typically saves
Across the plan ladders we hold, the three-month tier usually lands about 8–12% below the month-to-month rate, against 14–16% for six months and 16–20% for twelve. The marginal saving from three months to twelve is therefore smaller than the marginal saving from monthly to three months — you capture most of the discount at a quarter of the commitment.
The questions that decide whether it is worth it
- What is refunded if I stop mid-term for a clinical reason? Most compounded medication is non-refundable once shipped; the question is what happens to undelivered months.
- Does the rate hold at renewal, or revert? A discounted introductory term that reverts to a higher rate is a different product from a standing tier.
- Does the price change if my dose changes during the term?
- What happens if supply is interrupted for regulatory reasons? The FDA proposal on 503B bulk compounding is unresolved, and almost no provider addresses this in published terms.
Who it suits
Someone starting treatment who wants a better rate than monthly without betting a year on tolerating a drug they have not taken. Someone already established but unwilling to prepay twelve months while the regulatory position is open. And anyone whose provider's twelve-month discount is modest — where the gap between three and twelve months is a few dollars, the flexibility is worth more than the saving.
