Mounjaro vs Zepbound
Zepbound and Mounjaro are the same molecule — tirzepatide — approved for different indications. Zepbound is approved for chronic weight management, Mounjaro for type 2 diabetes. Lilly runs a self-pay programme for Zepbound at $299 to $449 and none for Mounjaro, which is why platforms list Mounjaro at up to $2,048 a month.
Same drug, different label
Tirzepatide is the active ingredient in both. The difference is the approved indication, the trial programme behind each approval, and the commercial arrangements around them. Chemically, a Zepbound dose and a Mounjaro dose of the same strength are the same drug.
The price difference is entirely commercial
LillyDirect operates a self-pay programme for Zepbound: $299 for the 2.5 mg starter vial, $399 at 5 mg and $449 from 7.5 to 15 mg, with the rate contingent on refilling within 45 days.
No equivalent self-pay programme exists for Mounjaro. That absence is why our dataset contains Mounjaro at $1,100 medication through one platform and $1,899 through another, reaching $2,048 a month all-in — the highest single figure in the entire dataset, for the same molecule available at $299 under a different label.
What this means practically
If you are being treated for weight management, Zepbound is the labelled product and the self-pay route exists. If you are being treated for type 2 diabetes, Mounjaro is the labelled product, and your prescriber's choice is a clinical one — but the self-pay economics are dramatically worse, and it is worth discussing.
Insurance coverage frequently differs between the two even though the molecule does not, because plans underwrite indications rather than molecules. A plan may cover Mounjaro for diabetes and exclude Zepbound for weight management.
What not to conclude
That they are interchangeable at the pharmacy counter. They are separate approved products with separate labels, and substitution is a prescribing decision, not a consumer one. The observation here is about pricing structure, not a suggestion to switch.
