Oral Tirzepatide and Sublingual Products: What Evidence Exists?
There is no FDA-approved oral tirzepatide. Compounded oral and sublingual tirzepatide products exist and cost $199 to $369 a month, with no trial evidence and unpublished bioavailability. Two approved oral GLP-1s do exist — orforglipron and oral semaglutide — both starting at $149 with published data.
The distinction that matters most
Approved oral GLP-1s exist. Approved oral tirzepatide does not.
Orforglipron (Foundayo) was approved on 1 April 2026 and oral semaglutide 25 mg (the Wegovy pill) on 22 December 2025. Both went through their own trial programmes for the oral formulation, because oral delivery is a genuinely hard pharmacological problem that has to be solved and then demonstrated.
Neither is tirzepatide. Products sold as oral or sublingual tirzepatide are compounded preparations that no trial has tested.
Why oral delivery is hard
An injected peptide bypasses the digestive system. Taken by mouth it faces stomach acid, digestive enzymes and poor intestinal absorption. Oral semaglutide solves this with an absorption enhancer, and requires administration in the morning on an empty stomach with no more than four ounces of water and a 30-minute wait.
Orforglipron sidesteps the problem entirely by being a small molecule rather than a peptide, which is why it has no food or water restrictions and is easier to manufacture.
A compounded orally disintegrating tirzepatide tablet inherits none of that engineering. How much drug reaches the bloodstream is the entire question, and for these preparations it is unpublished.
What the market charges
Compounded oral and sublingual products in our dataset run $165 to $369 a month. NexLife (opens nexlife.us in a new tab)'s ODT semaglutide at $165 and tirzepatide at $199 are the lowest; Henry Meds oral semaglutide at $249 and oral tirzepatide at $349; Shed sublingual formats at $199 and $229; MEDVi oral semaglutide at $369. These Henry Meds figures were taken from third-party publications and are withdrawn pending a capture of our own; see COR-009.
Every one of those is above the $149 starting price of an approved oral with published bioavailability and trial data. That is the clearest instance in this category of a compounded product costing more than the approved alternative while having no evidence of its own.
The episode that clarified the regulatory position
A large telehealth platform attempted to market a compounded copy of the Wegovy pill and withdrew under legal threat. FDA's stated objection is not to compounding generally — the agency acknowledges legitimate roles in shortages and for individual patient needs — but to compounding that reproduces an available approved product.
How to choose, honestly
If needle avoidance is the driver, the approved orals are the route with data behind them, at $149 to start and $25 to $50 for many insured or Medicare-eligible patients.
If price is the driver, the cheapest options in this market are compounded injectables, from around $147.
The compounded oral category is currently the hardest to justify on either axis, and it is worth saying that plainly even though several providers we cover sell it.
