Check coverage before anything on this page

The two cheapest rows are insurance routes, and they beat every self-pay option by a wide margin. Most people comparing compounded providers have never established whether they had a covered option.

Weight-management coverage is excluded far more often than diabetes coverage for the same molecules, and employer plans commonly carve it out as a category. The brand route is closer than it used to be

Brand Zepbound through LillyDirect is $299 at the starting dose and $449 at higher doses. That is an FDA-approved product with a published trial programme, and it is now within reach of several compounded programmes once they reach a maintenance dose.

One caveat worth knowing: the self-pay rates on the highest doses are reported to be contingent on refilling within a 45-day window, with the manufacturer listing $499 at 7.5 mg and $699 at 10, 12.5 and 15 mg outside it. If your schedule is irregular, that is a real risk to the saving.

Mounjaro is the same molecule and the wrong purchase

Mounjaro and Zepbound are both tirzepatide. The difference is the approved indication: Mounjaro for type 2 diabetes, Zepbound for weight management. There is no manufacturer self-pay programme for Mounjaro at weight-loss doses, and telehealth providers list it near retail at $1,100 to $1,899 a month.

If you are paying cash and want tirzepatide, Zepbound is the same molecule at a fraction of the price. Mounjaro makes financial sense only with coverage and a type 2 diabetes diagnosis.

Why this happened, in plain terms

We spent several pages criticising other publications for quoting prices without their term, dose and formulation. Then we republished their figures without checking any of the three.

Our evidence model has a status called reported, which means the provider reported it. We applied it to figures no provider had reported to anyone — they were things other websites had written. That is not a labelling nicety. It is the difference between a price and a rumour.

A new status now exists, third-party claim, and figures carrying it cannot be ranked, summed, averaged or compared anywhere on this site. The Embody case shows why: a single number, republished across at least five publications, that does not survive one search.

If you want a figure for any provider below, the honest answer is that you should read it on their own pricing page, on the plan you would sign, at the dose you expect to reach. That is what we did for the seven records above and have not yet done for the rest.

The question that settles it

Whatever route you are considering, one question does most of the work: what is the all-in monthly price at 10 mg, on the plan I would actually sign? Everything above is an attempt to answer that for a market where most providers do not publish it.
