Tirzepatide vs Liraglutide

Direct answer

Liraglutide is a daily injection; tirzepatide is weekly. Liraglutide is also substantially less effective — the SCALE programme reported roughly 8% mean weight reduction against about 20.9% for tirzepatide at 15 mg in SURMOUNT-1. It is not cheaper either: compounded liraglutide runs $119 to $179 against $147 to $215 for compounded tirzepatide.

The effectiveness gap is large

Across separate trial programmes — which is a weaker comparison than a head-to-head — liraglutide produced roughly 8% mean weight reduction in SCALE, semaglutide about 14.9% in STEP 1, and tirzepatide about 20.9% at 15 mg in SURMOUNT-1.

Those trials differed in population, duration and design, so the numbers are not directly comparable in the way SURMOUNT-5's head-to-head figures are. But the ordering is consistent and the gap is wide enough that the direction is not in doubt.

Daily against weekly

Liraglutide is injected daily. Tirzepatide and semaglutide are weekly. Over a year that is 365 injections against 52, which is a meaningful difference in adherence burden and in the practical experience of treatment.

And it is not cheaper

Compounded liraglutide in our dataset: Found at $169 on a twelve-month prepaid plan, Henry Meds at $179 month-to-month falling to $119 on twelve months paid in full. These Henry Meds figures were taken from third-party publications and are withdrawn pending a capture of our own; see COR-009.

So liraglutide sits inside the same price band as a more effective, less frequently injected alternative. On effectiveness per dollar and effectiveness per injection, it is difficult to construct an argument for it.

When it might still be the right choice

Clinical reasons exist and belong to a prescriber: tolerability differences, specific comorbidities, insurance formulary position, or availability where other agents are constrained. A daily dose also allows finer adjustment if that matters for a particular patient.

What does not follow is choosing it on price, because the price advantage is not there.
