What launched

TrumpRx went live on 5 February 2026, advertising direct-to-consumer access to lower-cost FDA-approved GLP-1 options. The listed monthly prices: semaglutide tablets $149 to $299 against a $1,349 reference, semaglutide injections $199 to $349 against the same reference, and tirzepatide injections $299 to $449 against $1,087.

Those figures sit alongside the manufacturers' own direct channels — LillyDirect and NovoCare Pharmacy — which publish comparable pricing.

Why the reference prices matter

The $1,349 and $1,087 figures are list prices, and almost nobody pays list. But they were the numbers that anchored three years of coverage, and they were the numbers the compounded market was positioned against.

A compounded programme at $250 a month reads very differently against $1,349 than against $299. The arithmetic that justified an entire industry has changed, and much of the published comparison content in this category has not been updated to reflect it.

What this means practically

Before buying any brand-name GLP-1 through a telehealth platform, price it at the manufacturer or through a direct channel first. Our pricing dataset contains brand Wegovy at $1,794 a month all-in through one platform when NovoCare sells the identical medicine direct for $349, and brand Mounjaro at $2,048 all-in when Zepbound is the same molecule at $299 to $449.

Those are disclosed prices rather than anything improper. They are also a fourfold to twelvefold difference for the same product from the same manufacturer, and the only thing standing between a patient and that difference is knowing to check.

The compounded comparison, honestly

Direct approved pricing does not undercut everything. The cheapest compounded programmes in our dataset run $110 to $186 a month, below the $149 approved oral entry price on some plans and below brand injectable pricing throughout.

What has changed is the middle of the market. Sixteen compounded offerings in our dataset cost more than $299, which is what brand Zepbound costs direct from its manufacturer. For those, the price argument has not weakened — it has inverted.

Where flat-rate pricing still competes

Dose structure is the remaining differentiator. Brand pricing tiers by dose: Zepbound at $299, $399 and $449 as the dose rises, with rates contingent on refill timing. A flat-rate compounded programme charges one price across covered doses.

Over twelve months at a maintenance dose, brand escalation reaches roughly $5,088 against about $2,232 for a flat-rate plan at $186. That gap comes entirely from structure rather than from the advertised monthly figure — which is why comparing at the starting dose ranks providers on a price nobody pays after month one.

What to check before switching

  1. Your insurance first. A covered brand can land near $25, below every cash route.
  2. The manufacturer's own channel, before any platform.
  3. The price at the dose you expect to maintain, not the starting dose.
  4. Refill timing conditions, which apply to several direct programmes.
