The change

NovoCare Pharmacy reduced its self-pay price for Wegovy maintenance doses to $349 a month from $499, effective November 2025. Starting doses and the oral tablet formulation are priced separately, with the Wegovy pill launching from $149.

The cut was one element of a sequence through late 2025 and early 2026 that reset the price floor for approved GLP-1s: LillyDirect's Zepbound self-pay programme at $299 to $449, the Wegovy pill at $149, Foundayo at $149, and direct channels advertising against list prices above $1,000.

Why manufacturers cut prices

Two pressures converged. Compounded alternatives had captured an estimated 30% of US GLP-1 supply at their 2024 peak at roughly $150 to $300 a month — a volume large enough to matter commercially. And regulatory action was closing that market, which meant capturing returning demand required a price patients would accept.

The result is a market where the approved and compounded price bands now overlap substantially rather than sitting an order of magnitude apart.

What $349 competes with

In our pricing dataset, compounded semaglutide runs $110 to $299 a month. So $349 remains above the compounded band for the same molecule.

The comparison changes on two axes. Against compounded tirzepatide, which runs $147 to $399, $349 for approved semaglutide is inside the band. And against the upper half of the compounded market generally, an approved product with premarket review and manufacturer supply at $349 is competing on more than price.

The figure that undercuts everything

Insurance. Where a commercial plan covers Wegovy or Zepbound, the cost can be roughly $25 a month. The Medicare GLP-1 Bridge is $50 for eligible Part D enrollees from July 2026.

Both sit below every self-pay route including every compounded programme in our dataset. The single most valuable thing a patient can do before comparing cash prices is establish whether they have coverage — and the exclusion is frequently indication-specific, so a plan that excludes weight management may cover the same molecule for diabetes.

What has not changed

Dose tiering. Manufacturer-direct pricing rises with dose, and several programmes attach refill-timing conditions to hold the lower rate. A patient at a maintenance dose pays materially more than the advertised starting figure.

That is the structural gap that flat-rate compounded pricing still exploits: one price across covered doses, which over a year at a maintenance dose can be less than half the escalating brand total. Whether that trade is worth making now depends on how a patient weighs an approved product against a cheaper unapproved one — a different question from the one the market was answering eighteen months ago.
